Endocrine therapy, typically with an aromatase inhibitor or fulvestrant plus a CDK4/6 inhibitor, is a mainstay for the management of HR+/HER2- metastatic breast cancer, but many questions still remain about the nuances of optimal use of these and other agents in first- and later-line settings. Furthermore, a number of investigational therapies are showing great promise in HR+/HER2- advanced disease, including novel ER-targeting agents such as oral SERDs, SERMs, SERCAs, PROTACs, and others, as well as antibody–drug conjugates such as those targeting TROP2.
